Insurance coverage for your Form Health program

This is a friendly reminder regarding our recent email about insurance coverage for your Form Health program. As outlined in the prior email, your insurer’s coverage has changed and will impact how we charge and bill for our program services. We understand that you may need more time to decide if you want to continue with our program due to this coverage impact, so we’ve extended the deadline for your response to July 9th. We're here to answer any questions you might have.

If you choose to move forward with our program, you will be transitioned to our $149/month* Form Health membership. Your membership fee covers:

  • All telehealth visits with your Registered Dietitian
  • Direct access to your Care Team via chat
  • Expert-led patient education and an extensive resource library.
  • Convenient digital food logging and tracking tools within our app, helping you to stay motivated and celebrate your wins.
  • Access to our supportive Form Health Community on Facebook.

** Please note: In addition to the monthly membership fee, all telehealth visits with your Form Health doctor, nurse practitioner, or physician assistant will be billed to your insurance, and you'll be responsible for any copays, deductibles, and coinsurance.*

Send a message to your Care Coordinator within the Form Health app to confirm that you’d like to continue by July 9th. If you have already informed your care team of your decision, please disregard this message. If you elect not to transition to the $149/month membership, we unfortunately will have to transition your care to a provider outside of Form Health. If this is your decision, we encourage you to contact your clinical team to discuss any medical needs during your transition of care. We highly encourage you to see another qualified physician. The American Board of Obesity Medicine has a directory on its website. If you do not respond, we will assume you are withdrawing from our program and you will lose access to Form Health on July 9th.
